This exciting opportunity has arisen to join our award-winning Specialist Palliative Care Service as a Macmillan Community Clinical Nurse Specialist. We are based in the beautiful rural country town of Midhurst and work in the surrounding locality in parts of Coastal West Sussex, Guildford and Waverley and South-East Hampshire CCGs caring for patients and their carers in their homes, nursing and care homes, and Community Hospitals.
The multi-disciplinary service works in an innovative way, offering interventions in the Community to optimize patient and carer experience and avoid acute admission, including drainage of Ascites, Blood Transfusion, IV Bisphophonates and Subcutaneous Hydration. We work closely with 19 GP Practices across the region, with the Community Nursing Teams and many other local agencies to provide collaborative, robust care.
Our service is hosted by Sussex Community Foundation Trust and so our staff have all the benefits of being employed by the NHS including favourable Agenda for Change pay scales and access to NHS Pensions. And we have the advantage of accessing the support , resources and education offered by Macmillan to its' post- holders.
We support our staff in further education and training either in-house, in neighbouring Services or via the local Universities and we fund our staff attending conferences. We offer on-going professional development support for Revalidation.
We are proud to report we received an outstanding rating for our responsiveness and good for the other domains in our CQC Inspection of 2014.
