An exciting opportunity has arisen to work within the new End of Life Care Hub in the Coastal area of West Sussex as a Telephone Referrals Coordinator. This new Hub will enable those facing ‘End of Life’ to choose where they want to receive care. The Hub will allow us, to ensure those facing end of life receive the best possible support that is right for their needs and the sensitivities of those close to them, regardless of their diagnosis and where they are being treated or supported. The Hub is a unique collaboration bringing together all the local services involved in palliative and end of life care.

This role will be part of a new team based in the Lancing Hub. The role will be primarily telephone triage, involving liaison and co-ordination of existing and new patients in their last year of life. Excellent communication and organisational skills are vital in this role alongside a desire and drive to improve and make a difference to end of life care in Coastal West Sussex. The post holder will work alongside End of Life Care Senior Nurse Assessors. If you have attention to detail and would go the extra mile we would love to hear from you.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
