We have a great opportunity for Senior Nurse Assessors to join our new Coastal Collaborative End of Life Hub ECHO.

We provide a single point of access for end of life co-ordination which provides triage, navigation and signposting to appropriate sources of care and support for patients in the last year of their life.

The aim of this service is to provide a responsive, dynamic, flexible partnership approach for patients registered to the service.

We are looking for people who have experience in community care in any setting, have excellent clinical and communication skills and want to help make a difference to patients who are in their final stages of life. Palliative care experience would be an advantage.

You will be passionate about end of life care with the ability to learn and develop as needed and have excellent organisational and communication skills. You will be part of an expanding team based in Lancing and the role will predominantly be one of telephone triage, liaison and co-ordination of existing and new patients in their last year of life.

This position will include shifts ranging between 07:00-1500 or 11:00-19:00 on a 4 week rolling rota. The service covers 7 days a week 24 hrs a day.
