We are looking for a Salaried GP to join our Community Hospital Team at Crowborough and Uckfield Hospitals.
Our community hospitals aim to provide comprehensive care to local people. This includes rehabilitation, medical management and end of life care. Patients are admitted from the community or from surrounding local acute Trusts.
You would be working in our 18 bedded unit in Crowborough and 14 bedded unit in Uckfield supported by our Advanced Nurse Practitioner. Geriatricians in reach weekly to support cross system working.
You will be the medical leader in the team, providing expert primary and community care for patients who require inpatient care but are not unwell enough to need admission to an acute hospital.
This unique post will allow you to combine your primary care knowledge with your passion and experience for more complex medical condition management with a chance to lead the care of our community hospital patients and develop and improve community medical services.
For this role you will have a proven clinical background with an interest in quality, improvement and service innovation. There is regular support in this role from Consultant Geriatricians and the SCFT Clinical Lead as well as study leave time and opportunities to attend medical education meetings. Candidates will also have opportunities to develop medical and clinical leadership skills on trust wide and health system wide projects.
Flexible working patterns including full, part time and job share will be considered.
You must have appropriate UK professional registration.
