Brighton & Hove Responsive Services & the IV Therapy Team are collaborating with an opportunity to work for not 1, but for 2 great teams. 6 monthly rotations between the teams, enabling practitioners to gain skills and confidence in both areas.
Based at Brighton General Hospital we deliver services across the city of Brighton and Hove within the patient’s normal place of residence and clinic settings.
You will Join our Multidisciplinary Team which consists of an Advance Nurse Practitioner, Nurses, Occupational Therapists, Physiotherapists, Registered Mental Health Nurses, Social Workers, Pharmacists, Community Reablement Assistants, Community Care Assistants. Working collaboratively with our acute Trust and other community services, including Third sector organisations we aim to improve the patient journey and pathways of care and deliver short term intervention, care and support in the community.
Responsive Services & IV Therapy Team delivers the ‘right care at the right time in the right place’ playing a key role in supporting patients discharge from hospital and community beds and support the patient to stay at home and prevent avoidable admission to hospital
